Gonoshasthaya Kendra University's Introduction

Gonoshasthaya Kendra emerged from the field hospital of Bangladesh Liberation War in 1971.During the last almost four decades, GK has increased its coverage from 50,000 people in 50 villages in 1972 to almost 1.2 million rural population with 39 Primary Health Care centres with 5 Referral hospitals two Tertiary Hospital, -covering 608 villages, 38 unions in 40 locations under17 districts across the country in 2010 making GK as one of the largest health care service providers outside the government of Bangladesh.

On Dec. 28, 2012, Specialists of Gonoshasthaya Kendra University, Bangladesh, Prof. Syed Anwarul Hafiz, Prof. Md. Shahidullah and Prof. Hasin Azhari visited Modern Cancer Hospital Guangzhou and started the 2-day academic exchange on medical technologies. Hospital Director Wang Huaizhong, international tumor specialist, Prof. Peng Xiaochi and other doctors and medical scholars gave them a warmly welcome.

In the morning of Dec. 28, specialists from Bangladesh visited hospital under the company of hospital Director Chen Bing. Director Chen first introduced them the Medical Technology Departments like Imaging Diagnose Center, Biochemistry Inspection Center, and Operation Room and so on. After that were the Service Facility Departments like International Patient Service Center, Café, wards in different styles and public kitchens. During the visit, the specialists of Bangladesh highly commented the medical devices and services in Modern Cancer Hospital Guangzhou.

Interventional therapy and related operation devices

At 9:30 of Dec. 28, a minimally invasive operation started in the operation room on the 2th floor of Modern Cancer Hospital Guangzhou. Bengali specialists under the arrangement of hospital leaders came into the interventional room to visit the operation devices and watch the procedures after changed shoes, put on masks and surgical gowns.

Communication between Bengali specialists and Prof. Peng Xiaochi

Prof. Hasin Azhari: 125I particles are a kind of radioactive specie of energy, will them take any effect on human body?
Prof. Peng: Though it is radioactive specie of energy, but the radiation range is within 1.7cm in tissues. In another word, as long as the particles are implanted in the parts that are 1.7cm under skin, they won't affect the normal tissues and people around patients.

Prof. Md. Shahidullah: Is it necessary to take them out after implantation?
Prof. Peng: The implantation of particles is permanent, so a patient doesn't have to take another operation to take them out.

Prof. Hasin Azhari: So the operation doctors should have good command of related technologies?
Prof. Peng: As radioactive particle implantation is performed under the guide of high-end imaging devices, before procedures a patient has to take multiple times of CT scan to confirm the location of tumor and the points for needle insertion. All those require a doctor who can handle imaging devices familiarly. To this patient, his tumor is in the middle of two blood vessels that the particles should be implanted carefully, so the demands of operation doctor are higher. Currently, this operation is difficult for many doctors to challenge. Specialist Liu has over 20 years experience in diagnosis and treatment of Radiology, his is familiar and skillful of this treatment.

Prof. Syed Andwarul Hafiz: How is the treatment effect of radioactive particle implantation in treating cancer patients? What about the survival rate?
Prof. Peng: Radioactive particle implantation is a persistent course to kill cancer cells. Normally, the necrosis of cancer cells can be clearly seen 2-3 months later than treatment. To the superficial tumors, we can see the tumor shrink gradually, when the tumors deep in body under the help of CT scan or Ultrasound B are found of size decreasing or necrosis. About the curative rate, there is none united answers clinically across the world. As different cancers in different stages, the correspondent treatment effect and prognosis are different. But we can sure that this treatment not only can extend the life of patient while it improves the living quality of patients.

Prof. Syed Andwarul Hafiz: Then this patient only has to take this radioactive particle implantation and that is enough?
Prof. Peng: No. Before this patient has this treatment, he already took a cycle of cryotherapy in the tumors of his left lungs. And after radioactive particle implantation, specialists will perform traditional Chinese medicine for physical regulation based on his condition. Only through combined treatments can reach the best treatment effect.

Prof. Hasin Azhari: How many particles are enough for him?
Prof. Peng: The quantity of implanted particles is depending on tumor size. The tumor is bigger the more particles are needed so as to reach the therapeutic effect. And during follow-up of patient, if the implanted quantity is not enough, we can implant more as supplement.

Prof. Md. Shahidullah: Is this treatment widely applied all around the world?
Prof. Peng: Actually the hospitals in the world that can successfully perform this treatment are not that many. But this technology is extensively used in USA. Even in China, there are only few hospitals can start this treatment.

      Apart from exchanging cancer treatment technology and other medical and surgical technologies, the Bangladeshi medical specialists held a more far-reaching purpose in their visit to Modern Cancer Hospital Guangzhou. That is to propose a nice vision of transnational medical cooperation with Modern Cancer Hospital Guangzhou. Both Modern Cancer Hospital Guangzhou and the visitors gave introductions of their own groups and their medical technologies, facilities and patient admission situation. In the exchanging meeting, both the host and guests discussed about the prospective cooperation areas with a hope that Bo Ai Group (BA) and Gonoshasthaya Kendra (GK) can share the advanced medical technology resources and medical talents and save patients together in the near future.
      Bangladeshi specialists had exchanging discussions with Deputy General Manager of Bo Ai Group, Tu Xiaoming, and President Wang Huaizhong as well as other administrators of Modern Cancer Hospital Guangzhou. In the communication, Head of Department of Medical Physics and Biomedical Engineering, Gono Bishwabidyalay of Bangladesh, Hasin Azhari, gave an introduction of the current medical condition of Bangladesh, showing a grave situation to all the attendees.
      First of all, medical institutions in Bangladesh are few. There are only 12 public medical institutions in Bangladesh, with only 5 hospitals possessing the basic equipments for cancer treatment and no one has set up an independent tumor department.
      Secondly, medical talents are in shortage. There are only 120 Radio-Oncologist in Bangladesh, which is far from the demand of 800 Radio-Oncologist. The number of Cancer Patients equals 2000 out of 1000'000 Inhabitants per Year.
      Thirdly, medical technology is comparatively backward, especially in cancer treatment as well as medical and surgical treatment for angiocardiopathy. So far, traditional methods are still the main treatment for cancer in Bangladesh.
      Lastly, as there is no national medical insurance to cover the treatment cost and the insurance is classified according to social status, many poor patients failed to get treatment due to financial problem.
      Dr. Hasin pointed out that, since 1971, Gonoshasthaya Kendra Group has set up many medical schools and medical institutions, held many charity activities and career training projects and organized disaster relief teams to treat patients and help citizens improve their heath and life standard. But all these are still far from meeting the demand of Bangladesh people. Regarding this, the visitors indicated that Bangladesh office of Modern Cancer Hospital Guangzhou has done a great favor to help solve these problems. They hope that Modern Cancer Hospital Guangzhou can give Bangladesh people more support in medical personnel education and training as well as medical technology, apart from that of Bangladesh office.
      As the response, President Wai Huaihong said that Modern Cancer Hospital Guangzhou has possessed international advanced cancer treatment technology and well-trained medical personnel. The hospital has organized many times the Southeast Asia tumor Forum and tumor conferences and made great influence in the Southeast Asia. Moreover, it has accepted nearly 40, 000 oversea patients through all kinds of channels. China and Bangladesh have established profound friendship long before. Hopefully this friendship can be enhanced through this medical bridge and more cooperation between two groups.
      Deputy General Manager of Bo Ai Group, Tu Xiaoming, also indicated that besides cancer treatment cooperation, Bo Ai Group is also willing to provide Bangladesh with support of cardiac operation technology and other high-end complicate surgical technologies. It is a great step forward of the cooperation between two groups that Chinese specialists held large academic forums in Bangladesh.
